Intermittent Fasting: Benefits, Risks, and Who Should Skip It

Intermittent fasting arrived in the mainstream around 2016 and has since become one of the most-searched diet strategies in the world. Its appeal is partly cultural — it is a diet that tells you not what to eat but when — and partly the promise, repeated in countless podcasts, that skipping breakfast will fix your metabolism, sharpen your brain, and possibly extend your life.

Some of that is true. Much of it has not held up under closer study.

What people mean by fasting

Several protocols share the label:

  • Time-restricted eating — the most common. Eating only within an 8-, 10-, or 12-hour window each day. "16:8" (16 hours fasting, 8 hours eating) is the popular version.
  • 5:2 — eating normally five days a week and restricting to about 500 to 600 calories on two non-consecutive days
  • Alternate-day fasting — alternating between normal eating days and fasting or very-low-calorie days
  • Extended fasting — 24 hours or longer, usually done occasionally

These have different physiological effects and different risk profiles. Most published research is on time-restricted eating and alternate-day fasting.

What the evidence supports

A reasonable read of the literature so far:

  • Weight loss: Intermittent fasting produces weight loss, but not more than standard calorie restriction when calories are matched. Its main advantage is behavioral: some people find it easier to skip meals than to count them.
  • Metabolic markers: Modest improvements in insulin sensitivity, blood pressure, and triglycerides in some studies, though these largely track with the weight loss itself.
  • Adherence: For a subset of people, particularly those who don't enjoy breakfast, time-restricted eating is genuinely sustainable. For others, it produces late-day overeating that cancels out any deficit.

A 2022 NEJM randomized trial in adults with obesity found that time-restricted eating produced essentially the same weight loss as daily calorie restriction over 12 months. A 2020 UCSF study found that time-restricted eating without other changes produced trivial weight loss and, worryingly, some loss of lean mass.

Intermittent fasting is a legitimate tool, but it is not a metabolic hack. It is calorie restriction wearing a different watch.

The claims about autophagy, longevity, and cognitive enhancement are largely extrapolated from animal studies and mechanism papers, not from robust human outcomes. They are plausible but unproven at the timelines most people fast.

Who should be cautious

Fasting is not neutral for everyone. Groups that should either skip it entirely or approach it with medical supervision:

  • People with a history of eating disorders — fasting protocols can reactivate restrictive patterns
  • Pregnant or breastfeeding people — energy and nutrient needs are elevated and consistent
  • People with type 1 diabetes or on insulin or sulfonylureas — hypoglycemia risk is real
  • Children and adolescents — growing bodies need reliable fuel
  • Older adults at risk of sarcopenia — insufficient protein spread across the day accelerates muscle loss
  • People training seriously for strength or endurance — performance and recovery often suffer
  • Anyone underweight or with a history of amenorrhea — additional restriction is the wrong direction

Women appear, in some studies, to have more variable responses than men, particularly around menstrual cycle regularity and sleep. The data are not conclusive, but the caution is reasonable.

What to actually do

If you want to try it:

  • Start with a 12-hour overnight fast (say, 7 pm to 7 am). This is close to how most people ate a generation ago and is unlikely to cause problems.
  • If that feels fine, try a 14- or 16-hour window. Notice whether you overeat at your first meal or feel worn out.
  • Keep protein high during your eating window — at least 1.2 g/kg body weight per day, spread across meals
  • Do not use fasting to compensate for poor food quality. A shorter window of ultra-processed food is not a healthy diet.
  • Stop if it makes you obsessive about eating, disrupts your sleep, or affects your mood or menstrual cycle

For some people, fasting is a genuinely useful structure. For others, it is a rebranded restriction that trades one unhealthy relationship with food for another. The protocol is less important than whether it makes your overall diet better or worse.
